What is the difference between
continuous flow and pulse flow oxygen delivery?
There are some machines on the market that offer both continuous and pulse flow. These machines offer flexibility as they are great for nocturnal use, use with sleep apnea equipment and during the day on pulse dose.
The continuous flow machine emits oxygen constantly irrespective of whether the user is inhaling or exhaling.Air compressor compress normal air and deliver high pressure air into oxygen system.
Oxygen system ( PSA system ) is made by two cylinders filled in molecular sieves.
It separates N2 and deliver O2. The bigger two cylinders, the more molecular sieves.
More molecular sieves need more compressed air. The more compressed air, the bigger compressor and more oxygen coming out per minute. That is why big home unit could produce 5 liters 93% oxygen per minute. Because big machine has big compressor and big Oxygen system.
Some cheap small potable units will mix normal air with oxygen when you turn up its flow above 1 liter.
The pulse flow POC provides a ‘pulse’ of oxygen each time you inhale. Oxygen concentration processing won’t stop no matter continuous flow or pulse flow.
Pulse flow machine take full advantage of people exhale time.
Pulse flow has a storage system inside of machine. When people exhale, storage system will stock oxygen and stop delivering oxygen out. There is a sensor to detect people inhale action. When people inhale, sensor will activate and deliver storage oxygen out.
Pulse flow top capacity equal to triple of continuous oxygen capacity, no matter how many settings this machine has. If oxygen concentrator biggest capacity is 1 liters continuous oxygen per minute with 90% purity, its pulse flow top capacity equal to 3LPM.
A POC oxygen device is ideal for people who are on oxygen therapy and want to travel.

Some airlines disclose a list of POCs and confirm the possibility to use them during flight. Some they provide contact information to confirm if your device is permissible.
When booking your reservation, airlines permitting the use of POCs or offering to supply oxygen, whether for a fee or at a cost. Airlines also require a minimum of 48 hours’ notice before travel to ensure approval for its use. In addition, some airlines restrict the number of passengers per flight who are permitted to use oxygen on board the aircraft.
It is highly recommended that patients requiring oxygen to book their flights early and contact their airline immediately to start the approval process. On most airlines, this is possible by calling, e-mailing or faxing their ‘Medical Assistance Service’ and completing the forms specified by each airline.
Each of the airlines has different policies for oxygen on board.

Many airlines permit patients to bring their portable oxygen concentrators on-board with no additional charges as long as they are on the Federal Aviation Authority’s (USA) list of approved POCs for usage.
• Liquid oxygen is prohibited for use on commercial airlines; the only aircraft permitted to carry or utilize liquid oxygen are helicopters serving as air ambulances
• Improper transport of either liquid or compressed oxygen can present significant safety risks resulting in operators being liable for large civil penalties.
The Department of Transportation (DOT) final rule “Nondiscrimination on the Basis of Disability in Air Travel” contains air carrier requirements regarding the use of respiratory assistive devices on aircraft.
In the DOT final rule, section 382.133 generally requires that:
Air carriers conducting passenger service must permit someone with a disability to use an FAA-approved portable oxygen concentrator (POC) on all flights (on aircraft originally designed to have a maximum passenger capacity of more than 19 seats). The device must meet applicable FAA requirements for medical portable electronic devices (M-PED) and display a manufacturer’s label that indicates the device meets those FAA requirements.

Before your flight with an oxygen device.
1) Visit your doctor to inform him/her about your trip. The oxygen needs on a flight might be different from the needs when you are on ground, because of the difference in the air pressure. Take a new prescription if necessary. Also inform the doctor if there are extreme weather conditions at your destination (high humidity, extreme cold, extreme hot, high altitude, air pollution). Weather conditions can make breathing even more difficult or/and can worsen your symptoms.
2) Make sure you have all the medicine you need.
3) Take at least one copy of your medicine and oxygen prescription and always have it on you.
4) Do a research for doctors and hospitals at your destination in case you need it.
5) We suggest you to have an oximeter on you to check your oxygen levels.
6) If you ‘re travelling abroad, check if your insurance covers you in case of emergency.
7) Make sureI that your Portable Oxygen Concentrator it has a FAA* approval.
8) Fully charge all your batteries.
9) Be sure you have the right power convertors/adaptors.
10) Take your cpap or bipap machine with you, even when you’ re not planning to stay the night.
11) Be prepared for possible delays and have extra batteries with you.
12) Be aware of the airline’s specific requirements regarding which oxygen concentrators are allowed and what paperwork is required.
Proper Packaging. POC users are responsible for ensuring that all POC batteries carried in carry-on baggage are properly packaged. POC providers, some airlines, and freight forwarders specializing in small package shipments may provide this packaging service for POC users.
1) It is always better (and safer) to travel with a companion.
2) Visit or talk to your doctor to inform him/her about your trip and the weather conditions there (humidity, temperature, altitude).
3) Make at least one copy of your medicine and oxygen prescription and always have it on you.
4) Do a research for doctors and hospitals at your destination in case you need it.
5) Make sure you have all the medicine you need.
6) Always have an oximeter on you and check your oxygen levels regularly.
7) Check your destination’s climate. High humidity, extreme cold, extreme hot and air pollution can make breathing even more difficult or/and can worsen your symptoms.
8) If you ‘re travelling abroad, check if your insurance covers you in case of emergency.
9) If you are using oxygen tanks, always have more than you will usually use.
10) ) Fully charge all your batteries.
11) Be sure you have the right power convertors/adaptors.
12) Take your cpap or bipap machine with you, even when you’ re not planning to stay the night.
13) Always be prepared for possible delays and have extra oxygen/batteries with you.
14) If you don’t have your own oxygen device or oxygen tanks, you can always rent. There are companies that can supply you with oxygen devices or tanks almost everywhere in the world.
During your flight with supplemental oxygen.
At the airport with a POC oxygen.
If you get breathless when walking, make sure you arrange for help at the airport. The distance between the check-in lounge and departure gates can be long. Disabled assistance at airports should be arranges at least 48 hours before you travel.
Inform the TSA officer that you have a portable oxygen concentrator and whether you can disconnect during the screening process. You may provide the officer with the TSA notification card or other medical documentation to describe your condition.
Consult your doctor to determine whether you can safely disconnect during screening.
Screening
If you are able to disconnect from the concentrator, you may submit it for X-ray screening so you may undergo screening through imaging technology or a walk-through metal detector.
If you must remain connected, your equipment will be tested for traces of explosives material.
Nebulizers, CPAPs, BiPAPs, and APAPs
A nebulizer, CPAP, BiPAP and APAP must be removed from its carrying case and undergo X-ray screening. Facemasks and tubing may remain in the case.
Flying with a Portable Oxygen Concentrator
Placement of Portable Oxygen Concentators.
The POC should be placed underneath the seat in front of the POC user so that the user or the user’s attendant can see the warning lights and/or hear the audible warning. Placement directly under the POC user’s seat and placement in a closed compartment would prohibit the user from seeing the warning lights, as well as possibly prohibiting the user from hearing audible warnings. Other placement locations may be acceptable.
When you are on the plane try to move every hour or so to exercise your legs. Drink plenty of water and non-alcoholic drinks during the flight.
POC Air/Intake.
In order for a POC to work efficiently, the air/intake filter must not be blocked during use. Therefore, the area around the POC should be clear of blankets, coats, and other pieces of carry-on baggage that may block the air/intake filter. If the air/intake filter is blocked, two things will occur. First, the POC user will be alerted by warning lights and/or audible alerts that the oxygen concentration in the POC output is insufficient. Second, when the temperature of the POC internal components increases to a certain limit because the POC is still trying to dispense oxygen, the POC will automatically shut down to prevent overheating of the POC and the POC user will be alerted by warning lights and/or audible alerts.
SEATING RESTRICTIONS FOR PASSENGERS WHO PLAN TO USE A POC ON THE AIRCRAFT.
Exit Row Seating. The FAA prohibits any person using a POC from occupying any seat in an exit row.
Stowage During Movement. During movement on the surface (pushback from the gate and taxi), takeoff, and landing, the POC must be stowed properly and in such a manner that it does not restrict passenger egress to any exit or the aisle in the passenger compartment. Additional seating restrictions may be necessary to comply with these FAA safety rules.
For example:
1. Some seats on an aircraft, such as bulkhead seats, may or may not have approved stowage space to accommodate a POC during movement on the surface, takeoff, and landing. Therefore, the POC may not be able to be stowed properly during these phases of flight if the POC user occupies those seats. In this case, a seating restriction would be required to comply with an FAA safety rule.
2. During movement on the surface, takeoff, and landing, the tubing that is used to dispense oxygen from a properly stowed POC to the user’s mask/nasal cannula may stretch across the row in such a way as to restrict passenger egress or become a tripping hazard in an evacuation.
The POC user must not restrict another passenger’s egress during these phases of flight. In this case, a seating restriction may be required to comply with an FAA safety rule. For example, if all seats in the row are occupied, the appropriate seat for the POC user would be a window seat.
However, if there are no other passengers in the row, or if there is one other passenger in a row of three seats and that passenger is seated in the aisle seat, or if the POC is stowed in such a way that the tubing does not block another passenger’s egress, then other seats in that row may be appropriate as long as no other passenger’s egress is restricted by the tubing.
3. An operator can only establish seating restrictions based on an FAA safety rule. The examples above represent some, but not all, scenarios to consider. The operator must make a safety decision based on the specifics in an individual situation before establishing a seating restriction.
Note: A general airline policy that all passengers who board the aircraft with a POC must occupy a window seat, without regard to the specifics of the individual situation, would be inconsistent with the requirements of part 382 (refer to § 382.87(a)).
USE OF POCs DURING THE LOSS OF CABIN PRESSURE.
Cabin Depressurization.
There is no danger posed by a POC that is operating during a loss of cabin pressure.
In the case of loss of cabin pressure (rapid or slow), POCs typically will not continue to meet the oxygen needs of the user at cabin pressure altitudes above 8,000-10,000 feet. This is because the lower ambient air pressure at higher altitudes makes the concentration of the oxygen output of the POC too low to meet the POC user’s oxygen needs.
In cabin depressurization, the POC user should be instructed to discontinue use of the POC and use the oxygen masks that deploy to provide supplemental passenger oxygen until the aircraft descends below 10,000 feet cabin pressure altitude.
Limitations of Portable Oxygen Concentrators.
Passenger briefing requirements require crewmembers to instruct passengers on the necessity of using oxygen in the event of cabin depressurization. Each POC user will receive this briefing with the rest of the passengers.
Oxygen POC users who are routinely dependent on their POC as their primary source of supplemental oxygen may not recognize the limitations of their POC or that the depressurization procedures in the standard passenger briefing also apply to them. Operators may wish to emphasize this important depressurization procedure to POC users.
Typical operator procedures require that the aircraft rapidly descend to an altitude where supplemental oxygen use is no longer needed after a loss of cabin pressurization.
Because of limiting factors such as high terrain, the aircraft’s descent may have to be halted at a minimum en route altitude resulting in the aircraft’s cabin pressure altitude staying above the cabin pressure altitude at which supplemental oxygen is needed for passengers, and above the cabin pressure altitudes below which POCs have demonstrated optimum performance (typically below 8,000-10,000 feet). In this case,
F/As should follow SOPs regarding the use of decompression first aid oxygen when addressing the additional oxygen needs of POC users. F/As should also know that all POCs will again meet the oxygen needs of the user when the cabin pressure altitude reaches 8,000-10,000 feet or below.
The POC user is responsible for the operation of the POC on board the aircraft. For this reason, the FAA recommends that passengers carry with them any POC operating instructions from the POC manufacturer or other written information provided by their healthcare professional regarding their POC use.
Battery damage and battery short circuit can result in battery overheating and fire. These events, in turn, can result in personal injury to passengers, and in the worst case for certain types of batteries, a catastrophic passenger compartment fire. Thus, spare lithium batteries carried on board aircraft must be individually protected from short circuit by placement in original retail packaging, by otherwise insulating terminals by taping over exposed terminals, or by placing each battery in a separate plastic bag or protective pouch. Note: Spare lithium batteries are prohibited from being carried in checked baggage on an aircraft.
1) Portable Oxygen Concentrators (POC) and Stable Concentrators can be a little noisy so if you’ re planning to stay in a hotel, motel etc. inform during your booking that you will have supplemental oxygen device in your room.
2) For your convenience ask for a room near the elevators or at the ground floor.
3) Some hotels provide supplemental oxygen, so ask during your booking.
4) The room that you will stay in, must be clean and dust free.
5) Chlorine is bad for the lungs, so ask during your booking or at the reception to clean your room with chlorine free products if it’s possible.
6) Avoid to swim in indoor pools. The smell of the chlorine might intensify the symptoms of your lung disease.
7) If you have arranged for a rental oxygen device or oxygen tanks/cylinders to wait for you at you, your accommodation contact the supplier to inform about your arrival.
8) Use the power supply as much as possible.
9) Charge your batteries whenever you can.

Patients face a number of additional challenges with respect to arranging the use of portable concentrators. Patients typically must provide at least 48 hours’ notice of intent to travel, together with a physician statement for how and when the POC is to be used during flight. Many airlines do not guarantee a power outlet for use of the POC and instead recommend ensuring the machine is fully charged and that additional charged batteries are also carried. Finally, some airlines require patients to assume all risk by signing a waiver of liability before air travel.
It is important therefore that physicians be well informed on such challenges faced by patients, to provide them with the best information available. One speculates that a physician–patient discussion akin to informed consent might be appropriate under certain circumstances given the waiver of liability that the patient is expected to sign.
Respiratory Disease–Specific Recommendations
Patients with Interstitial or Restrictive Lung Diseases
A detailed assessment should occur as described previously. In addition, if traveling to a high-altitude destination, supplemental oxygen should be considered. The patient should also bring a course of oral corticosteroids and antibiotics in the event of an acute exacerbation of their disease.
Patients with Neuromuscular Disease or Chest Wall Disease
All patients should undergo assessment as described previously as well as hypoxic challenge testing.
Patients with Cystic Lung Disease
In addition to hyperinflation within communicating airways, at an altitude of 8,000 feet, Boyle’s law predicts there will be a 38% increase in the size of closed air-filled pockets within the body. This gas expansion may be associated with an increased risk of pneumothorax in patients with bullous or cystic lung disease . However, from published data in patients with cystic lung disease, the incidence of pneumothorax related to air travel appears to be low. Nonetheless, in patients with chronic lung disease who are already at risk of hypoxemia, the development of a pneumothorax in-flight could be devastating. A previous history of pneumothorax may be more relevant in patients with lung disease, as rapid changes in barometric pressure may precipitate recurrence.
Air Travel in Patients after Pneumothorax
Patients with a closed pneumothorax generally should not fly, with rare exceptions. It is currently recommended that patients not travel after a pneumothorax unless drainage has been performed and, in the case of recurrent pneumothoraces, when definitive surgical treatment has occurred. Otherwise, provided chest imaging has determined the pneumothorax has resolved, patients should be safe to travel provided a further 7 days has elapsed. However, it should be noted that patients with preexisting lung disease have a high risk of recurrence and that this risk remains increased for up to 1 year after a pneumothorax.
Air Travel after Chest Surgery
Similar to the advice pertaining to pneumothorax, in those who have undergone chest surgery, radiographic demonstration of lung reexpansion and resolution of pneumothorax after chest drain removal should be performed . Signs or symptoms concerning for pneumothorax or postsurgical complications should prompt further investigation before air travel.
Preflight Optimization of Pulmonary Status
In all of the previously described conditions, it would be prudent to recommend that patients not fly while symptoms of an acute exacerbation of their illness may be developing.
In contrast to this scenario, there may be some role for optimization of their pulmonary illness before flying, thus decreasing the risk of complications and potentially reducing the severity of hypoxemia. A period of more intensive chest physiotherapy may be of benefit to some patients with cystic fibrosis, for example. Similarly, optimized control of asthma symptoms, or symptoms of heart failure, would also seem to be appropriate recommendations, although there is no published evidence to support this.
Patients for Whom Air Travel Is Contraindicated
Certain patients with pulmonary disease should be instructed not to fly. These include patients who pose risk to others such as those with active infectious diseases (e.g., tuberculosis or influenza), or those in whom air travel would pose a risk to themselves: hemoptysis, unresolved pneumothorax, and a sea-level supplemental oxygen requirement in excess of 4 L/minute.
Additional Considerations When Evaluating Patients for Air Travel
It is important to highlight the following points in assessing patients for air travel:
Even at 35,000 feet, different types of commercial aircraft will have widely differing cabin altitudes, ranging from an equivalent of approximately 5,400 to 8,000 feet (4). In addition, commercial aircraft may also vary their cruising altitude a number of times during the flight, which in turn can alter cabin pressure.
Respiratory symptoms may occur even despite having a preflight assessment. One study found 18% of patients with COPD developed respiratory symptoms despite having a preflight evaluation.
Flight duration is another important factor to consider. Longer flight durations are associated with increased symptoms , particularly when lasting over 3 hours.
The levels of activity of the patient during the flight should also be considered. Patients with COPD, restrictive lung disease, and cystic fibrosis demonstrate significant worsening of hypoxemia at simulated altitude with a workload equivalent to that of walking around the aircraft cabin.
Conclusions
In summary, normobaric hypoxic challenge testing should be considered for patients with chronic and acute pulmonary diseases as they are potentially at risk of significant hypoxemia and complications during air travel. In addition, patients with comorbid illness, at risk of significant complications in hypoxic environments, particularly if compensatory mechanisms might be inadequate to deal with hypoxemia, should be assessed for fitness to fly. These also include patients with significant cardiac or cerebrovascular disease. Furthermore, when prescribing supplemental oxygen, consideration should also be given to the anticipated oxygen concentrator and delivery mode and also levels of patient activity aboard the flight.

Advances in CPAP technology now make it possible for patients with sleep apnea to use their CPAP devices during long-duration flights that span normal sleeping hours.
Like Portable Oxygen Concnetrators, CPAP machines are classified as medical assist devices. They are, therefore, permitted on most domestic and international flights.
